How you will be notified:

You will be contacted either by text, or a letter from the NHS.  If you have not yet been called, you can book an appointment by going on the NHS UK website   (you will need your NHS number) or calling 119 anytime between 7 a.m. and 11 p.m.  Please do not contact the surgery.  With regard to your 2^nd appointment, if you have not heard 1 week before your appointment is due, use the same method as before to arrange this 2nd jab.  Policy is to return to same place as you had your first vaccine.

Venues:

Vaccination was previously offered offered at Congleton Town Hall in Phase 1, or if you book on-line you will be offered other venues close by.   Phase 1 has finished,  – “Watch this space” for new information.

The NHS has not provided patient transport services to vaccination centres. Patients who cannot drive themselves are being encouraged to seek support from family or friends to get to their appointment.
